Our Impact

Thanks to Together Toward Health funding and support, grantees have reached millions of Californians across the state. Our partner organizations are committed to connecting all of California’s diverse communities with information, vaccinations, and critical resources for COVID recovery. From creating unique outreach approaches and training community health workers, our grantees’ work has paved the way for a stronger, more resilient public health system.

“It is a perfect example of how to partner between public health departments and nonprofit community-based organizations in order to really create an accelerated, advanced model for how to engage.”

Genoveva Islas, Executive Director, Cultiva La Salud (Fresno)

Awards by Region

Our grantees are breaking down barriers that prevent people from getting the help they need, providing information to community members in multiple languages, and using innovative outreach activities to build trust and gain the confidence of diverse residents.

Through TTH funding, 540 community organizations are doing critical work across the state, including in specific communities in Northern California, Central California, Southern California, and with Tribal and Native American populations.

Regional totals may add up to more than the total number of grantees due to organizations that work in multiple regions

In San Diego, Refugee Leaders Find Success Together

There are only two markets that sell injera bread in all of San Diego County. An important staple food for the Ethiopian and Eritrean community, this tangy, sponge-like flatbread is served for practically every meal, alongside flavorful spiced meat and vegetable dishes.

When County and Community Expertise Combine

When most people think of Santa Barbara, they often picture palm trees and the Pacific coastline, seafood restaurants and maybe even Oprah, a well-known denizen. But the County of Santa Barbara, with a population of almost 450,000, is much more varied and diverse than its most recognizable town.

For Native Communities, Cultural Reflection is Key

Virginia Hedrick distinctly remembers the day in March 2020 when she returned home from a work trip to her family and friends panicking about the looming pandemic. As a Native public health professional and concerned mother, she knew she had to do something to address people’s fears. That very night she sat at her kitchen table and logged on for her first Facebook Live stream about COVID-19.

“The most significant outcome of this funding was that for the very first time in history, it really brought together all the organizations that served the LGBTQ community. And for the very first time, we really got to sit together on a table and really brainstorm and strategize toward improving the health and the wellbeing of the entire LGBTQ community. We're really grateful for that.”
